Program Description

All individuals living in one of Thresholds Homes and Supports housing options receive support services. Housing options range from group living, with 24 hour/seven day a week support, to shared living where support levels vary depending on the individual's need. Housing is available in various locations throughout Waterloo Region, Wellington, and Dufferin Counties. The model of service is recovery and person-centered. Intake is continuous. Wait times vary but can be lengthy, depending on availability.

Access Information

Referrals to this program are made through Here 24/7. While an initial request for services can be made by someone other than the applicant, the process always involves the person ensuring that decisions reflect the person's choices.
