Assertive Community Treatment Teams

Program Description

For those 18 and above who have complex bio-psychosocial needs due to severe and persistent mental illness. Intensive treatment, rehabilitation and support include; assertive outreach, 24-hour crisis service provided in conjunction with Sault Area Hospital Crisis Services, supportive counselling, medication monitoring, skills building and advocacy. Model of service is assertive community treatment.

Access Information

Must meet ACT criteria. High prior hospitalization, emergency, and/or community involvement. Priority given to transfers from other ACT teams, long-term psychiatric units and schedule one facilities.
